How to Get a Medical Marijuana Card in Ohio

Ohio's medical marijuana program was established through House Bill 523 in 2016 and began serving patients in 2019 after a regulatory development period. The program is jointly administered by the Ohio Board of Pharmacy, which manages the patient registry and dispensary operations, and the Ohio Department of Commerce, which oversees cultivation and processing licenses. With recreational cannabis legalized through Issue 2 in November 2023 and sales beginning in 2024, the medical program continues to provide valuable benefits for qualifying patients.

The tax advantage for medical cardholders in Ohio is straightforward and significant. Recreational cannabis purchases are subject to a 10 percent excise tax on top of standard state and local sales taxes, while medical purchases are exempt from the excise tax. This creates immediate savings for patients who purchase regularly, making the $50 annual registration fee a worthwhile investment.

Ohio's qualifying conditions list is one of the more extensive in the country, covering cancer, HIV/AIDS, post-traumatic stress disorder, epilepsy and seizure disorders, chronic pain, fibromyalgia, Crohn's disease, multiple sclerosis, Parkinson's disease, sickle cell anemia, Tourette syndrome, traumatic brain injury, hepatitis C, inflammatory bowel disease, and several other conditions. Medical patients are also permitted to grow up to 6 plants per person at home.

The registration process begins with an evaluation by a physician who holds a Certificate to Recommend. Telehealth evaluations are widely accepted, making the process accessible across the state. After the physician submits the recommendation, patients register through the Ohio Board of Pharmacy portal and pay the $50 fee. Processing typically takes 7 to 10 business days, and approved patients can purchase from any licensed dispensary in the state.
